2025 Senior Jersey Order Form
Use this form to submit your order for your 2025 Senior Jersey.

Jerseys are $50 and come with "SENIOR" on the front,  "CHS" on the right sleeve, and "2025" on the left sleeve.
Customize the back of the shirt for $1 per letter (capital letters and digits 0-9 only).
2XL add $2. 3XL add $3.

A 2XL shirt with "UN1CORN" on the back would cost $50 + $2 + $7 = $59

Make your check, cashiers check, or money order payable to "Chestatee High School". If you are paying with cash please bring the exact dollar amount - no change will be given. NO ORDER WILL BE MADE WITHOUT PAYMENT.

After filling out this form, please turn in your payment to Darrell Skogman in Room 1116.

PAYMENT IS DUE BY FRIDAY, AUGUST 16. Jerseys will be available for pickup in mid-September.
