This form is to register for Greater Harvest Global Ministries, Inc. NA/ME, Never Alone/ Mind Empowerment, Singles Day Out on Saturday, September 19, 2020 at Axes and Armor Hatchet House.
Each participant is required to register separately for the outing.This event registration is $35.00 per participant and is welcome to all adults ages 18 and up only. Registration includes entry to Axes and Armor Hatchet House and participating in a two hour game. All attendees must provide transportation to and from GHGM. Full payment of $35.00 per participant is due at time of entry at Axes and Armor Hatchet House.
Departure from GHGM will be at 12:45PM and safely returning to GHGM by 3:30PM. All registrants will be responsible for their own transportation.

Participant of NA/ME Singles Outing: by completing this registration in its entirety, this means you will be participating in the event and agree to all terms expressed in the liability waiver.
Liability Waiver:
The specified registered attendee is participating in activities and or attending services and or events as a participant of his/her own free will of Greater Harvest Global Ministries, Incorporated.
I, the attendee, hereby waive for myself, my heirs, executors, administrators or assigns, any and all claims, demands, actions or causes of action, against Greater Harvest Global Ministries, Inc., its officers, agents, volunteers, and employees, of whatever kind or nature may arise in any manner by reason of injury or damage to me or property or both while I am participating in the activities, services, and or events.
I, the attendee, do hereby agree that I will not instigate any suit or action against Greater Harvest Global Ministries, Inc., its officers, agents, volunteers, and employees, for damages or loss or injury of any kind for or on account of any damages, loss or injury to me or property or both which may arise in any manner while participating in the activities, services or events.
This agreement holds harmless Greater Harvest Global Ministries, Inc., its officers, agents, volunteers, and employees, for any injury, including but not limited to claims for wrongful death, arising in any manner to me while participating.
I, the attendee, have read the foregoing liability waiver and vow not to sue and understand that this constitutes a formal legal document.
