Request for Handicapped, Disabled, or Hearing Impaired Seating Vouchers for Graduation
Each graduate will receive 5 tickets for this year's Graduation Ceremony on Friday, May 31, 2019 at 2:30pm at DAR Constitution Hall.  Graduates do NOT need a ticket, but all other people attending the ceremony including infants are required to have a ticket to enter Constitution Hall.  

If guests need handicap accessible seating or hearing impaired accessible seating, requests can be made for that person and ONE accompanying person to be seated in the handicapped seating areas of the Constitution Hall.  THESE VOUCHERS DO NOT REPLACE TICKETS, anyone with a voucher must ALSO have a ticket to enter Constitution Hall.  Parents/Guardians may request handicapped seating/hearing impaired seating vouchers for the QOHS Graduation ONLY by completing this form.  Vouchers will be included with the tickets which will be distributed following Graduation Rehearsal on May 30th.
