IUSD Summer School 2021 - Device Needs
If you are enrolled in IUSD or IPSF Summer school and wish to extend your checkout of an IUSD device, please submit one copy of this form per student that is enrolled.  Most secondary students are being allowed to hold onto their devices over the summer.  

If your school has asked you to return your Chromebook, this form will extend the due date of your device and add you to our notification list for our July return dates.  We'll need you to please verify the Barcode number or Serial Number of your Chromebook so we can update that device's records and know you won't be returning it to us at this time.
