This is a proxy/ballot form for the 2023 Board of Directors Election and is open to all residents of Odessa National with the exception of those who live in Legacy/Southerness.

This proxy/ballot is valid for a period of one year and can be used for any future meetings pertaining to the election of the Maintenance Corporation Board of Directors for 2023.

If you have already mailed in a ballot/proxy, you do not need to complete this form.

Owner(s) of the property address listed above being in attendance at the annual election meeting of the Corporation set for November 3, 2022 (or any subsequent meeting pertaining to the 2023 election of the Odessa National Maintenance Corporation board of directors) and being member(s) of the Corporation under the provisions of Article III, Section 6 of the bylaws, do hereby submit this ballot to cast my/our votes as follows:
