El Dorado MS Family Compact - Spring 2021

Mt. Diablo Unified School District is committed to providing a safe, inclusive, and welcoming environment for all students, staff, and families. We are calling upon the entire community to help us maintain the safety and health of our schools through responsible actions as we seek a metered return to in-person instruction. Our goal is to maintain a variety of learning environments, all employing the use of consistent health, physical distancing, and sanitization procedures. In order to provide a safe and respectful learning environment, we request that all Mt. Diablo Unified School District students, staff, and families pledge to follow the guidance outlined below. We are collectively responsible for the common good of our local community and greater world. More information is available in Mt. Diablo’s Re-Connect Safely, Reconnect Strong Plan.

Before students and families arrive at school each day, we agree to do the following: (please check the boxes acknowledging your agreement) *

Perform a self-assessment of COVID-19 Student Home Screening Tool for symptoms and remain at home if any symptoms are present (congestion/runny nose, nausea/vomiting/diarrhea, sore throat, headache, fatigue, muscle/body aches, cough, difficulty breathing, loss of taste/smell, fever of 100° F or higher).

Alert the school site immediately if a student, staff member, or close family member tests positive for COVID-19 OR has been exposed to someone with COVID-19 symptoms or a confirmed/suspected case.

Consult with a healthcare provider about testing recommendations if a student, staff member, or close family member has had close contact (closer than 6 feet for more than 15 minutes within 24 hours) with someone who has COVID-19.

Abide by county and state guidance for physical distancing, mask wearing in public, avoiding large gatherings or gatherings where attendees are not practicing appropriate safety measures, and maintaining appropriate handwashing and hygiene practices.

Maintain physical distance 6 feet between teachers and staff or students and adults.

Wear cloth face masks that cover the nose and mouth at all times.

Wash or sanitize hands appropriately when they arrive on campus and frequently throughout the day (before and after eating, using the restroom, etc.).

Avoid sharing food, supplies, materials, etc.

Follow individual school procedures for drop off, pick up, and moving throughout the campus.

Avoid higher-risk travel outside the Bay Area while you are working/your student is learning on campus or 14 days before a scheduled return to campus.

Obtain testing via your own healthcare provider or county health department.

Observe routine medical practices, e.g., routine annual checkups, immunizations, and flu vaccines (It is highly recommended that everyone gets a flu vaccine this year).
