In order to receive diapers or feminine products, you MUST do the following. If all items are not complete you will NOT receive diapers.
YOU WILL NEED TO FILL OUT A FORM FOR EACH DIAPER APPOINTMENT. EACH CHILD WILL NEED AN APPOINTMENT. THEY CAN BE ON THE SAME DAY AT THE SAME TIME.
1. NEW MEMBERS and RENEWING MEMBERS must pay the Membership Donation of $10 per child per year.
2. PHOTO ID
3. MEDICAID CARD(s) Your card and cards for all children receiving diapers.
4. Complete our Information form
5. Complete our Demographic survey
