2024 Leota 8th Grade Party: Parent Consent and T-shirt Size Request
Hello parents of 8th graders! 

Even if your child will not attend the 8th Grade Party, they still get a T-shirt! Please fill out this form to tell us your child's T-shirt size. 

The Leota PTSA is using this form to collect the following information to help us prepare for the 8th Grade Party on June 18, 5:30-8:30 pm, at Brightwater Center:

  •   Parent/guardian consent to attend
  •   Preferred size of your child's Class of 2028 T-shirt (Leota PTSA's gift to all 8th graders!)
  •   Emergency contacts
  •   Food allergies and dietary restrictions

A student who does not have this form on file will not be allowed into the event.

Submit this form and pay by Friday, May 17, to receive early-bird pricing for the 8th Grade Party. Ticket price: 

  •   $25 per ticket if purchased by May 17
  •   $35 if purchased on or after May 18

Financial assistance is available--please see a school counselor. We want every 8th grader to have the opportunity to celebrate this milestone with their friends!

Submit one form per student. If you have multiple 8th graders, please fill out a separate form for each child.

3. Every 8th grader will receive a T-shirt celebrating the Class of 2028. This shirt is free to all 8th graders, compliments of the Leota PTSA. One shirt per student.

What size would your 8th grader like? (Tip: Submitting this form by May 17 will give you the best chance at receiving your preferred size. After that date, your request is not guaranteed.)

4. Will your child attend the 8th Grade Party? Event will take place Tuesday, June 18, at 5:30-8:30 pm at Brightwater Center in Woodinville. *

5. If your 8th grader will attend, please read the following statement about the 8th Grade Party and type your digital signature below. Your student will not be allowed to enter if we don't have this signed form on file.

"The 2024 8th Grade Party will take place at Brightwater Center in Woodinville on June 18 at 5:30-8:30 pm. Parents/guardians are responsible for transportation to and from Brightwater. Once a student leaves the event, they will not be allowed re-entry. Students must be picked up by 8:30 pm. This event is only for currently enrolled 8th grade students of Leota Middle School who also meet all of these criteria: 1) They have a signed Consent Form on file (aka this form), 2) They have paid in full or have received a scholarship to attend, and 3) They show a valid and current school ID at entry. For safety purposes, no backpacks or large bags will be allowed inside. Leota PTSA is not responsible for lost or damaged items. Students are expected to conduct themselves according to school standards. If a behavior issue arises, the child's parent/guardian will be called and instructed to pick up their student immediately."

By typing your full name in the space below, you grant permission for your child to attend the 2024 Leota 8th Grade Party. Your digital signature is equivalent to your handwritten signature in this online form.

6. Please provide the name and phone number of 2 adults we can contact in the event of an emergency. If there is a medical emergency, we will call 911 to evaluate your child.

7. Does your child have food allergies or dietary restrictions that we should be aware of as we plan the menu?
8. Is there anything else you'd like event organizers to know about your child (e.g., medical conditions, restrictions, sensitivity to strobe lights, etc.)?
9. Next steps: Please submit this form, then purchase a ticket at www.leotaptsa.com. Registration is not complete until we receive payment. Cost: 

  - $25 per ticket if purchased by May 17
  - $35 if purchased on or after May 18

If you prefer to pay by check, please make it payable to "Leota PTSA" and drop it off in the Leota PTSA mailbox in the office.
