By filling out this form and entering the spectator area at Phil Moore Park for the 2020 KHSAA Region 2 AAA Cross Country Championship, you are agreeing to properly wear your mask AT ALL TIMES, even when 6 feet apart from others. This is a requirement for us to have you as a spectator at this event.

I agree, that I do not exhibit any of the well known COVID-19 related symptoms including: fever (100.4+), cough, sore throat, shortness of breath, fatigue, muscle aches, loss of taste/smell, vomiting or diarrhea. *
I agree that I have not been in close/direct contact with, nor have cared for someone with COVID-19 in the past 14 days. I also agree that I have not tested positive for COVID-19 within the past 14 days or I have been cleared by the health department for a positive COVID-19 test within the past 14 days. *
I agree that I will not enter the spectator gate for this event Friday, October 23, 2020 if my responses noted above have changed due to me experiencing COVID-19 symptoms or me coming in close/direct contact with (or caring for) someone who has COVID-19.  I also agree to properly wear a mask or face covering AT ALL TIMES, not just when entering or exiting the spectator area and to practice social distancing. I understand that I will be asked to leave the spectator area if I refuse to follow these requirements. *
