By submitting this form, you are requesting to check out a Chromebook from Irons Jr. High for school use. You understand that the Chromebook will be checked out to you in working order and that you are responsible for returning it in working order. You are responsible for any loss or damage to the Chromebook. The Chromebook is valued at $226 and the charger is valued at $35. Cracked screens cost $50 to replace. Your submission of this form will count as your signature, accepting these terms.
