As part of our safeguarding policy, we need to ensure that children are only collected at the end of the school day by an adult that the parent or carer of the child has agreed with the school can collect them.

Therefore, we are asking that all parents and carers complete the following form stating the people that can collect your child at the end of the day. Please note that anyone collecting a child must be over the age of 14.

If there are any changes to what you have submitted on this form or there is an occasion where you need someone other than the people that you state below to collect your child then please contact the school office to let us know.
