All Disciples, Lukers, and Volunteers on behalf of students need to complete this form BEFORE coming onto the Luke 18 Retreat. (The night before)

The screening protocol outlined below is based on a review of screening protocols from multiple agencies, recommendations by the CDC and a literature review of the most common signs and symptoms of COVID-19.

Any information collected on this screening will only be accessible by SCLYM Youth Ministry Team,  medical personnel in order to satisfy the needs for contact tracing.
