Cannon River Sustainable Farming Association Mentorship Program
Please fill out the application to be considered for a year long business-to-business mentorship program. The mentorship program will be facilitated by the Cannon River chapter of SFA and mentors will be selected from fellow farmers/business-owners in the 7 counties served by the Cannon River chapter (Dodge, Goodhue, Steele, Rice, LeSueur, Dakota, and Scott).

By filling out a mentorship application you agree to fulfill the following expectations:

• I currently am or I will become a member of Cannon River SFA.
• I will participate in the mentorship program for the full 12 months.
• I will create SMART goals with my mentor to guide our time together.
• I will communicate at least once a month with my mentor via phone or email.
• I will have my mentor visit my farm or business at least twice during the season.
• I will visit my mentor's farm or business at least twice during the season.
• I will communicate with the Cannon River SFA periodically throughout the year via emails, phone, or by responding to written surveys.
• After the 12 months, I will provide feedback about the mentorship.
