Birthday Party Inquiry Form- Park City  
We are excited to celebrate your child's birthday party at Black Diamond Gymnastics! Please fill out this inquiry form and a member of our party crew will reach out to you within 2 business days. All birthday parties include the following: A coach/coaches (depending on how many guests) that will be there to take care of everything! They will have your party room ready prior to your event, greet all attendees, have a fun hour in the gym planned for your group with safety and fun being our top priority! Then they will help you serve food and clean up so you don't have to worry about a thing. We want to make this stress free for you! All parties come with utensils, napkins (white), paper plates (white), tablecloth (color of your choice) and a balloon for each attendee. If you would like to bring your own themed decor/ plates and tablecloths please indicate that on this form. 

Your special birthday child will receive a t-shirt as a thank you for celebrating with us!
