Introduction
This questionnaire serves as your application to the CPP training and was designed to help you think about your and your agency's readiness to take part in the CPP Learning Collaborative (LC) and implement CPP.  



Note: Learning any model requires time and either an adjustment to productivity expectations or an agreement by participants that they will do some of the learning on their own time.

INDIVIDUAL PRACTITIONERS
We typically do not accept private practitioners to training as we feel that learning to do a trauma treatment requires the support of a team. If you are in an agency with an existing team, and they can support your learning, we are happy to have you join.

Some trainers may offer exceptions if the  following conditions are met:

1) practitioners reside in a rural area or an area where there are limited services for young children

2) practitioners provide services to low-income families who have experienced trauma

3) practitioners are licensed

4) practitioners agree to form reflective supervision consultation groups at least for the duration of the 18-month learning collaborative.

YOUR LEARNING COLLABORATIVE TEAM
Number of staff expected to participate in the current LC:
A learning collaborative team is comprised of (1) at least one agency administrator (who has the power to implement agency policy) (2) at least one supervisor, and (3) one or more therapists. The supervisor must be licensed, and the supervisor/agency administrator can be the same individual. If indivuduals in your agency have already completed a CPP training, they can serve in those roles, and you do not need all roles filled for the current LC
Each person should be included in only one category (the one that best fits them). If you have team members who are in or who completed a prior CPP implementation-level training, you will enter them in the next section.

Minimum requirements for 18-month training:  Clinicians treat at least 4 CPP clients. Supervisors who want to be rostered - at least 2 CPP clients. Children are aged 0-5 and experienced at least one traumatic event.

Note: As an agency begins to provide trauma treatments, it is critical that agency staff (senior leaders, supervisors, and therapists) think jointly about how trauma may affect the therapist when they assign a caseload.

AGENCY REFLECTIVE PRACTICE FIDELITY
CPP requires that team members have regular time  to reflect on the model, their work, and their own reactions.

During training, all CPP participants meet at least twice a month (for at least one hour each time) for CPP supervision/consultation within their own agency (this is separate from the consultation they receive with the CPP trainer).

NOTE:
* Participants who are not in an agency (e.g. private practice clinicians) should form CPP peer supervision teams.
* Agency and peer supervision/consultation groups are often conducted on the weeks when a CPP consultation session is not held with the CPP trainer.
