I acknowledge that this athletic event could be a test of a person’s physical and mental limits and carries with it the potential for injury or hurt. I hereby assume all of the risks of participating in this event. I certify that I am physically fit and have not been advised otherwise by a qualified medical person not to participate in sporting activities. In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) Waive, release, and discharge from any and all liability from this event, West Rouge Basketball, employees and volunteers, (B) indemnify and hold harmless all entities or persons mentioned in this paragraph from any and all liabilities or claims made by other individuals or entities as a result of my actions during this event. I understand that at this event or related activities i may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the event holders, sponsors, affiliate or organizers. This waiver shall be construed broadly to provide a release and waiver to the maximum extent permissible under the applicable law. This waiver also covers for photo days and all photo taken when parents are involved in the activity on court or in a Game with children or during photo day. I hereby certify that I have read this document and I understand its content.

GENERAL TERMS AND CONDITION *

If registration link is still accepting athletes please go ahead and sign up

Once the required number of athletes are signed up, the link will stop accepting athletes

Athletes are not to come with their basketballs to the gym

Athletes are to ensure they make 80% of the Practices

Basketballs for drills and Games will be provided by the club

Parents are responsible for athletes drop off and pick up

Athletes are to be responsible for their items while on the gym.

Athletes and parents are responsible for athletes' belongings and safe keeping and closure of their water bottles.

Waiver and consent form already signed covers for all home Games and away Games. when applicable.

Waiver and consent signed at registration covers for all tournaments that athletes desires to partake of , a new consent is not required.

Waiver and consent cover for all programs by the Club or its affiliate that players agree to participate in .

Another waiver will not be given for programs within our network that players desire to attend

Registration is non-transferable

Games will not hold on days where gym is not available due to holidays, strikes or permits or weather conditions that are beyond the clubs control.

Parents and caregivers are responsible for their safety during any session and are to protect themselves as best as they can from any risk that accompanies a sports event with kids involved and any accident that may occur to parents or caregivers who are in the gym space.
