We will include this information on their nametag, so our volunteers are aware.

Please note, if the participant requires feeding or toileting assistant, our volunteers are not allowed to perform these tasks. Staff will need to see to the participants needs.

Participant (or parent/guardian if Participant is a minor) gives Shalom CRC permission to use Participant’s name, likeness, image, voice and other identifying information in pictures, video broadcast or livestream, video recordings, audio recordings, digital images, statements (written or oral) (“Content”) and that Content may be used in Shalom CRC’s promotional materials, on its website and social media, contained in livestreamed events or otherwise broadcasting of the Content on the internet, and any other form.

I have been given a full explanation of the Activity, and acknowledge that participation in the Activity involves risk to the Participant (and to Participant’s parents or guardians, if Participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage and financial damage (“Injury or Damage”).In consideration for the opportunity to participate in the activity described above (the “Activity”), the Participant (or parent/guardian if Participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the Activity, as applicable. The Participant (or parent/guardian if Participant is a minor) accepts personal financial responsibility for any Injury or Damage sustained during the Activity or during transportation to and from the Activity. In addition, Participant (or parent/guardian if Participant is a minor) grants permission to Shalom CRC or its agents, employees, volunteers, or any other representatives (collectively referred to hereinafter as the “Activity Sponsor”) to authorize medical assistance and/or treatment to Participant and Participant (or parent/guardian if Participant is a minor) agrees to be financially responsible for all such medical assistance and/or treatment. Further, the Participant (or parent/guardian if Participant is a minor) releases and promises to indemnify, defend, and hold harmless the Activity Sponsor for any Injury or Damage, including attorneys’ fees incurred by Activity Sponsor arising directly or indirectly out of the described Activity or transportation to and from the Activity, whether such Injury or Damage arises out of the negligence of the Activity Sponsor, the Participant, or otherwise.

The Participant expressly agrees that this agreement is intended to be as broad and inclusive as permitted by the laws of the State of South Dakota, to continue for as long as permitted by the laws of the State of South Dakota, and that this agreement shall be governed by and interpreted in accordance with the laws of the State of South Dakota. The Participant agrees that in the event that any clause or provision of this agreement shall be held to be invalid, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this agreement which shall continue to be enforceable. If a dispute over this agreement or any claim for damages arises, the Participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the Participant (or parent/guardian) and the Activity Sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution pursuant to the rules of the American Arbitration Association.

I understand that all participants who live in a staff house must have 1 staff member per house present inside the Shalom Christian Reformed Church building from the time the participants are brought in until they return to their house transportation. I understand that it is the job of the houses to provide safe transportation to and from all events. I understand that all feeding, toileting and behavioral issues for participants living in a staffed house must be handled by the staff present for that house.
