This form has been updated to reflect data ownership and usage criteria. This form is a survey to collect basic information about you, including your interest in participating in outreach requests and preferred communication methods, as well as your outreach efforts up until now. To report outreach for activities (going forward from January 2024), please use the PANDA Outreach Reporting Tool or maintain your own spreadsheet.
There are five sections in this survey. Please fill out the areas relevant to your work.
- Basic information including contact preferences
- Direct/Indirect and Formal/informal outreach
- Data Ownership/Use criteria
- Mentoring, recruiting etc.
- Everything else and non-peer reviewed media
We would like to have as much information as you can share, but if you can report even for the past year or two, that would also help us better understand the state of current Education and Public Outreach (EPO) efforts in the department.
