Event Release and Waiver | Liberación de Eventos y Renuncia

In the consideration of the permission granted by The Arc Jackson County to participate in activities of The Arc Jackson County, the undersigned hereby releases and waives any and all claims which could be asserted against The Arc Jackson County and their officers, agents and employees (the ‘Released Parties’) by the participant/volunteer, his/her personal representative, heir(s), on account of injury to or death of the participant, or damage to the property of the participant arising out of the participant’s/volunteer’s participation in activities at The Arc Jackson County.

Authorization for Medical Care and Consent to Agreement | Autorización para atención médica y consentimiento para el acuerdo

In case of injury or illness, appropriate staff will contact the parent or legal guardian of the participant. If the parent or legal guardian is unavailable, appropriate staff will contact the emergency medical contact(s) provided for the participant. The undersigned hereby consents and authorizes appropriate staff to take any steps deemed necessary in the event of any medical emergency. The undersigned agrees to be personally responsible for the costs of any emergency transportation and/or medical care deemed reasonably necessary, and waives and releases any claim relating to such care that could be alleged against The Arc Jackson County. The Arc Jackson County shall have the right to refuse or terminate participation in the event that such participation shows symptoms of any contagious illness (flu, cold, COVID, etc.) at the time of the program.

I hereby authorize The Arc Jackson County and its legal representatives and assigns, the irrevocable and unrestricted right to use and publish photographs and film, videotape, or other rendering of me, or in which I may be included, for editorial, advertising, and electronic publishing and any other purpose and in any manner and medium, to alter the same without restriction as the The Arc Jackson County may determine appropriate.
In giving this consent, the undersigned also releases The Arc Jackson County, from any liability, which may arise out of the broadcast, publication, or other use of said photographs, film, videotape or other recording.
