All Students in Music House Dance Classes have the opportunity to showcase all that they have been learning in a Dance Recital. We would like to know if your child will NOT be participating.

Recitals have a $75 Participation Fee that will be processed via the payment method on file on your Music House Account. We will process the fee on January 15th for the Recital.

If you wish to NOT participate in the next performance, you will need to complete this form before the January 15th processing date so we know not to charge your account.

If you enrolled in a dance class between the above processing date and a performance date your dancer would be ineligible for participation in the performance.

*Please note that if you are enrolled in more than one dance class you will be charged per costume, however the maximum charge will be for two costumes total.
