I have created this form as a way to intake information regarding our proposed shoot that is vital for the purposes of planning as well as providing a rate for my services. In order to provide an accurate rate, I need to know as much detail about the intended project as possible.

Here's a very quick bio (assuming you are not familiar with me or my work):

I have over a decade's worth of experience in various types of modeling: I have done everything from nude figure modeling in studios, to staged bridal portraits, to glamour/editorial concept work, commercial video work, indie film, fashion blogging, to window storefront live modeling. I have been published five times in boudoir and glamour magazines. As a former makeup artist, I can provide my own basic hair and styling unless the project calls for something more advanced, as well as basic wardrobe when necessary. I tend to choose my modeling work very selectively as I strive to create a cohesive aesthetic in my portfolio.

As with every project, quotes are dependent upon concept, location, and other factors. This will be negotiated on a case by case basis, but I work around a ~$125/hour basis. I am willing to consider work on certain projects with budgets, so please answer the below question accordingly with your budgetary requirements or limitations.
