感謝您在百忙之中抽空參與這項研究，本問卷的目的在於探討馬來西亞男性遭遇性騷擾之知覺與態度，您所提供的資料僅作學術研究分析之用，絕不做其他用途；且是作整體性的分析，並不會針對個人做單一性的解釋。此問卷除了研究者以外，不會有任何人看到您的答案，加上是不記名方式作答，因此，請您放心地填寫。

這份問卷沒有所謂的對或錯的答案，請依據您個人的意見作一個表達。它包含了三個部分，每一個部分均有詳盡的說明，請您事先詳細的閱讀說明之後在填寫問卷。第一部份為問卷填答者的個人基本資料（9題），第二部份為性騷擾知覺調查（20題），而第三部分為性騷擾態度調查（26題），共計55題，請您仔細回答全部的問題。

Hello gentlemen:

Thank you for taking time out of your busy schedule to participate in this research. The purpose of this survey is to explore the perceptions and attitudes of Malaysian males when they encounter sexual harassment. The information you provide is only for academic research and analysis, and will never be used for other purposes. This is a holistic analysis, and will not pinpoint on a single survey. This result will not be seen by anyone except the researcher, and you will answer anonymously, so please fill it out at ease.

This questionnaire has no right or wrong answer, please answer based on your personal opinion. It contains three parts, each part has detailed description, please read the description thoroughly before filling out the questionnaire . The first section is the basic personal information of the respondents (9 questions), the second section is the sexual harassment perception survey (20 questions), and the third section is the sexual harassment attitude survey (26 questions). It is a total of 55 questions, please answer all the questions carefully.
