PTA Club Proposal
This form should be completed by any staff member/parent/community member who would like to run an after school club at Fremont.  Adult to student ratio will be 1:15 for all clubs, and a background check is required for any non-staff members running a club without a staff member present. If you are a mandatory reporter, you do not need to complete a background check, but will be asked to verify your credentials.

You will be notified when your club is approved. Approvals will be made 4 sessions at a time.  For staff who qualify for reimbursement, there will be a $50 max per session. Please add a description in the comments for special circumstance where more may be needed. Please contact Anna.LH@gmail.com with any questions.
