MDH PERT for Dual Enrollment Qualification Test Request
Please complete all sections of this form to request a review of your eligibility to take the PERT assessment for Dual Enrollment qualification. 

This request is for the May 21 test opportunity.

Sign up early! May 10 is the deadline but sign-ups will close early if test requests exceed our capacity to test.

To be eligible for PERT testing, students must have an *unweighted* GPA of at least 3.0.

While PERT does not have any time limits, students should expect to complete all testing in a single day/session. There are no make-up dates for PERT for Dual Enrollment except in extenuating circumstances.

Sub-test scores required to dual enroll are:
PERT Reading - 106
PERT Writing - 103
PERT Math* - 114 for Intermediate Algebra or 123 for College Algebr1a
