Daily COVID-19 Screening for Visitor and Vendor Access
You are required to complete the Alliance University Daily COVID-19 Screening for Visitor Access. You should complete this form prior to entering the campus. 

Masks are not required. Regardless of vaccination status individuals who are asymptomatic, and have been identified as a close contact of a confirmed case for 10 days after exposure continue to be required to wear masks on campus. 
The information you provide may also be used to monitor and respond to the presence of the COVID-19 on campus. The information will be maintained in accordance with all applicable laws and public health guidelines.
