This form is a request for enrollment into the St. Maurice Sacramental Preparation Program. Our program is for grade 2 parishioners of St. Maurice parish as well as grade 2 students at St. Gregory School. Once this registration is reviewed, you will be notified by September 29th that your child is either prepared for the program and able to proceed, or that further information is required.
