Please complete one form for EACH INDIVIDUAL dancer.

Performance dates are December 9-11. Dancers will be assigned a cast around the end of October but must be available for any performances that weekend.

For each date listed below, please list times you are ABSOLUTELY UNABLE to rehearse & WHY - being specific. It is not necessary to list morning conflicts - only conflicts after noon. All conflicts MUST include a time frame (ex. work - 5:00pm-10:00pm), or indicate that it is an all day event (ex. sister's wedding - all day).  The following conflicts should be listed: work, graded schoolwork, major family life events (ex. sister's wedding). PLEASE DO NOT LIST any social or sports related activities. We are unable to schedule around extracurricular activities.

You must still call/email the absence coordinator to be excused from rehearsal even if it was listed on this sheet. The absence coordinator can be reached at assistantdirector@balletmidwest.net or 215-1792. The absence policy will be reviewed in more detail at a later date. In a typical season, dancers who start rehearsals the first two weekends are allowed 2 excused absences. Dancers who begin rehearsals in late October/early November are allowed 1 excused absence. Please plan to save absences for illness this year.
