Informed Consent Form
Dear participant,
The following survey is performed as part of a research in startups project funded by the Cyprus Research Innovation Foundation (RiF). The research is on active sports tourism, which refers to individuals that travel to another country to actively participate in a sport event. The results of this survey will be used for the enhancement of SportsTravelers76 (https://www.sportstraveler76.com/) services and will be published anonymously in the form of reports and research papers based on the survey. We will not ask you to provide personally sensitive data in this survey. All information collected via this survey will be kept confidential. The confidentiality of the information will be safeguarded subject to any legal requirements; my data will be password protected and accessible only by the named researchers and company members.

By responding to this questionnaire, you confirm the following:
- I have read and understood the purpose of the survey.
- I understand that my taking part is voluntary. I can withdraw from the study at any time during the survey and I do not have to give any reasons for why I no longer want to take part.
- I agree that the answers I give will be stored in digital form. Only the involved researchers and the company's personnel will have access to this information and this information will not be distributed to any other person or organisation.
