In any physical activity, risk of serious physical injury is possible. Yoga and other activity is no substitute for medical diagnosis and/or treatment. The student assumes the risk of yoga or other activity and releases the teacher and facility from any liability claims.

My child is participating in classes or workshops with Sarah Freemyer of SF Yoga & Wellness LLC, during which time they will receive information and instruction about yoga. I am aware that yoga requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand that it is my personal responsibility to consult with a doctor prior to and regarding my child’s participation in any physical fitness program, including yoga. I represent and warrant that my child has no medical conditions that I am aware of, which would prevent them from taking part in yoga classes or workshops, and I assume responsibility for any risk or injury they may sustain as a result of their participation.

In consideration of being permitted to participate in the yoga classes, I knowingly, voluntarily, and expressly waive any claim I may have against the instructor, the owner, or the leaseholder of the building for injuries or damages that my child may sustain as a result of participating in classes or workshops held by SF Yoga & Wellness LLC. I acknowledge that I have read the above release and waiver of liability and understand its contents.
