Volunteer Dental Clinic Application Form
Thank you for your interest in Volunteering with Quest's Volunteer Dental Clinic!

Quest has two fully equipped and recently updated dental operatories that are available to provide basic dental care for registered uninsured Quest clients who would otherwise not have access to dental care. Clients (ages 19-64) who have aged out of Healthy Smiles and are not yet of age or qualify for the Ontario Seniors Dental Care Program (OSDCP).

Since 2013, Quest’s Volunteer Dental Clinic has provided over 3,000 visits to clients at no charge.

This was only made possible by Volunteers and the 6,100+ hours they contributed to ensure Quest clients had access to over $656,000 worth of dental services. Together with our volunteers we continue to remove barriers to dental health care and continue to make dental health a vital part of our primary health care practice.
