Sacred Heart After School Club (Smart Play)
This registration form is to be completed by ALL children who would like to attend the After School Club run by Smart Play at Sacred Heart School. From September 2023, this is the only method of booking and one form must be completed per child.

After School Club runs everyday until 6pm with three different sessions:
  • 3.15 - 4.30pm - £8 
  • 4.30 - 6pm (after a club) - £9
  • 3.15 - 6pm - £12 
After you have completed this registration form, please EMAIL our administrator Dawn on info@smart-play.co.uk to confirm the dates of your booking and the total cost.

Please note that for insurance reasons your child/children will not be accepted on any of our sessions if they have not completed the booking process above.

Section 3 - Collection and Lateness
Sessions finish at either 4.30pm or 6pm and children should be collected from the main door at these times (or before if more appropriate). 
Please list the adults/older siblings you authorise to collect your child. For safeguarding reasons, permission for collection by any adults NOT on this list must be emailed. Please list up to 3 authorised adults with their name and relation to child.  *
My Year 6 child is allowed to travel home on their own.

Lateness 

If you book the early session until 4.30pm, a £4 charge will be incurred for late collection after 4.30pm. The £4 is the difference in price between the 4.30 and 6pm sessions. 

If you book the full session until 6pm, a charge of  £1 per minute of lateness starting from 6.05pm will be incurred. All money will go directly to the staff required to stay out of their hours on that date.

Section 4 - Payment Terms and Cancellations
Sessions are to be booked and paid for either as a block booking (whole term/half term) or as ad hoc sessions. For a full term booking, booking is made once at the start of term. For ad-hoc bookings, please email 48hrs before with your booking. If it is less than 48hrs, please email us and we will try to accommodate (latest 2pm on the day). Booking emails are checked and replied to at various points throughout the day. 

In all cases, payment is expected BEFORE the session commences (termly booking or ad hoc). If you need support with making payments, please get in touch to discuss payment plans.

Payments are non-refundable if your child cannot attend a session due to sickness or other last-minute changes. You will be charged for the days pre-booked, even if your child is unable to attend. Fees will only be refunded in exceptional circumstances and at the discretion of management.

If you need to make changes to your booked days or session lengths, please email as soon as possible.

Payment can be made in a variety of ways. To support our admin, how would you like to make payment? *
As we are Ofsted registered, we are able to provide confirmations of payment statements to submit to Universal Credit or similar organisations. Please advise if you will need a payment statement.

Section 5 - Consent 

Medical Consent

I consent to any emergency medical treatment necessary during the running of the After School club. I authorise Smart Play staff to sign any written form of consent required by hospital authorities if the delay in obtaining my signature is considered by the doctor to endanger my child’s health & safety.
