Parents or guardians, please check the 'yes' box below if you agree that your child can attend Serve a thon 2023, and fill out the additional information requested below. You can include all of your children on this form.

-- There is no cost for this trip.
-- Dress code is their Serve a thon shirt (they will get this the morning of) and jeans and sneakers.
-- Transportation method will be ACSD vans or parent-driven vehicle.

*If your child is under 8 years old and is traveling off campus with their class PA law requires a booster seat. Please send it with your child on the day of the trip. Students will return to BCS or WCA before the end of the school day and will take their usual transportation home.
