Please take a moment to complete the following form regarding your family's intent to return on campus for the current 2020-21 school year. We need this information to finalize our proposed reopening plans within guidelines from CHDP and current COVID-19 trends and data.

The District has developed a number of possible instructional models which include days of on-campus instruction as well as days of at-home virtual instruction contingent upon complying within constraints provided by OSHA and CHDP as well as the number of students who will be on campus.

Per Governor Newsom's orders, we aim to provide our students with as much in person instruction as possible when it is safe to do so.

There are givens regardless of which model the district moves forward with:

--Masks will be worn by all individuals both inside and outside at all times
--Six foot distancing will be maintained both inside and outside
--Lunch will not be served on campus. Students who receive free and reduced lunch will take their lunches home
--The school day will be shortened and will last around 4 hours and 15 minutes
--Students will have a structured break outside with their stable group
--Parent/Caregivers will be responsible to screen students for illness and COVID symptoms prior to departure for school
--COVID-19 mitigation measures such as tracing, tracking and notification, regular disinfection,  and quarantine for individuals exposed to COVID-19 will all be in place in accordance with CHDP guidelines.
