Request to Participate in Pilot Program
This summer, research labs, studios, and core facilities have the opportunity to participate in a pilot program on reopening that will inform campus policy for the fall semester.

This form should only be completed by each research director or designated assistant or lab member. Please note, a designate may complete the form; however, it is the director's responsibility to decide if moving to this phase.

If updates to the lab roster need to be made throughout the summer, please resubmit a new form.
