Please complete this form in its entirety. This form is for students 17 and under to apply and enroll in the Lighthouse Project. Upon receipt of this form we will reach out to the student and parent/guardian with additional information and a start date of participation.

Audio Arts Club and Visual Arts Club take place every Monday and Wednesday, 4pm-7:30pm. Dinner is included. Consistent attendance is encourage but not required.
