NSAI - OKC Chapter Meeting Permissions
Song Submissions and Consent Statement for Peer Feedback Listening

At times when we are unable to conduct our monthly chapter meetings in-person and we meet using the Zoom meetings platform, we will use the following form to collect permission and information before-hand.  You do NOT need to fill out this form if you will not be presenting a song.

"NSAI thanks you for participating in our programs and services. We are one of the few organizations that take time to listen and evaluate your songs. Due to legal considerations, we require you to sign a consent form any time NSAI hears your song (whether it be through the Song Evaluation Service, Pitch to-Publisher Night, Feedback Night, One-on-One Counseling, Monthly Chapter Meetings or any one of NSAI’s special events and services). Your submission of a song (whether lyric, instrumental, or music and lyrics) for critique/feedback by NSAI constitutes your agreement to release, indemnify, and hold harmless NSAI and its employees, board of directors, officers, attorneys, agents, and representatives from any and against any claim, including, without limitation, of copyright infringement or other misappropriation, arising from any acts or omission in connection with your submission." *

How would you like to present the music and lyrics of your song?  You can either send links to or attachments of your files to coordinators in advance at Bryan@BryanMitschell.com, or you can chose to present them yourself from your own computer/device by either playing it live or sharing your computer sound with the group.  Please note that the sound quality of your song, when presenting it yourself, will depend heavily on the audio equipment you are using and your personal competency with the Zoom meeting software.  Please consider reviewing this online video tutorial to learn more: https://www.youtube.com/watch?v=50NoWIiYECA) *
