It is time for our Annual Awards and Installation Banquet plus membership renewals!

Please complete the bottom form as well as payment to renew your IBM Ring 21 Membership and secure your Installation Banquet Reservation!

Event Timing: October 7th, 2021. Social Time Starting at 6:30 pm and Dinner at 7:00 pm PT

Event Address: Smoke House Burbank - 4420 Lakeside Dr, Burbank, CA 91505

Everyone is welcome! This is a great opportunity to bring friends and family members to an IBM Ring 21 event!

The menu is: Tri-Tip Beef, Marsala Chicken, or a Vegan Pasta dish. (Please indicate which choice you would like while completing the below form).  

Following dinner there will be a wonderful Magic Show featuring a Surprise Performer!

There will be a raffle at the banquet with multiple prizes that we have acquired over the year special for this event! If you pay your dues by October 7th 2021 you will receive 3 free raffle tickets for the Installation Banquet at the event!

Finally, payment will be made outside of this form but should be completed in tandem. Payment may be made via Check, PayPal or Venmo.

This years rates are as follows:
Junior or Adult Renewal is $35.00
Adult + Spouse Renewal is $40.00
Installation Banquet is $36.00

So, for example if you are renewing your membership and bringing one family member to the banquet it would be: $35.00 for the renewal + $72.00 for Two Banquet Reservations for a total of $107.00
