As the legal parent or guardian of child/camper listed above, in registering my child to More Than A Song Studios The Spirit of Song Summer Camp I hereby;

1. Give consent for my child to participate in all aspects of the camp program.

2. I understand that the camp hours are July 15 - 19, 2019 from 9am - 3pm.

3. My child does not have to participate in the activities if she/he does not feel comfortable or confident doing so.

4. I certify that my child has no undisclosed medical or physical condition which could interfere with his/her safety, or else I am willing to assume – and bear the costs of - all risks that may be created, directly or indirectly, by any such condition.

5. I understand and accept that More Than A Song Studios/The Neat Coffee Shop/Rising Lotus Yoga are not responsible for lost, damaged or stolen property brought to camp by myself or my child/dependent.

6. I understand that More Than A Song Studios/The Neat Coffee Shop/Rising Lotus Yoga cannot accommodate campers with known life-threatening allergies. I understand that More Than A Song Studios/The Neat Coffee Shop/Rising Lotus Yoga cannot guarantee a “Nut Free” environment and may not always monitor food products for traces of specific allergens.

7. I understand that More Than A Song Studios/The Neat Coffee Shop/Rising Lotus Yoga reserves the right to terminate any camper when it is deemed to be in the best interest of the camper or the camp.

8. I understand that snacks and lunch are included in camp costs. Campers are welcome to bring along their own snacks and lunch in the case that they do not like what is being served.

9. I understand that More Than A Song Studios/The Neat Coffee Shop/Rising Lotus Yoga will approach this camp with both care and planning. While the camp is underway, they will endeavour to instruct, protect, and care for the well-being of my dependent as would I in their place.

10. Information is gathered for the purposes of registration, processing payment, and ensuring camper health and well being during the program. The information on this document is held as confidential. More Than A Song Studios/The Neat Coffee Shop/Rising Lotus Yoga will not give my information to third parties (unless with my permission or required by law).
