CWSCS Summer Program 2022 Registration Form

Use this form to register your student for the full day CWSCS Summer Program, 8:30am - 3:00pm. You will need to fill this form out for each student you are looking to register. Summer Program will be offered Monday through Thursday beginning Tuesday, June 21, 2022 and ending Thursday, July 28, 2022. There are no programming options on Fridays.

**Student participation in Summer Program will not be allowed unless the total registration is paid in full.** Summer Program is $400. This completed, submitted form and deposit of $100 are required by May 27, 2021; the full balance is due Friday, June 17, 2022.
