Expanded Learning Opportunity Program                              Interest Form 2023-2024

Hello Fruitvale Families! 

As we plan for the upcoming 2023-2024 school year, it is time to share your interest in our Expanded Learning Opportunity Program. ELO is a free program. This after-school program includes grades TK- 6th and operates every school day from the release of the regular day until 5:15 pm. Behavior expectations are congruent with the regular school day, and attendance is required Monday - Friday. 

Before submitting this interest form, please click on the link and review Fruitvale's ELO Handbook. It is essential to read the entire handbook to understand participation requirements. Please complete a form for every child you hope to enroll as soon as possible.

I understand that the ELO Program operates from regular day dismissal until 5:15 pm. Your student(s) are expected to stay until 5:15 pm daily.  Grades TK-2 dismiss to ELO at 2:15 pm, and grades 3-6 dismiss to ELO at 3:05pm. 

Early pick-up will count as an unexcused absence, and multiple early pick-ups may result in dismissal from the program.

I understand that the ELO Program does NOT offer transportation; therefore, your student(s) must be picked up by a parent/legal guardian at 5:15 pm. 

There are no exceptions to this policy. Students will not be allowed to walk home, and those picking up will be asked for identification until they are known.

I understand that the ELO Program operates on ALL regular school days beginning, tentatively, August 15, 2023, through May 30, 2024.

On a minimum day, all students report to ELO at 12:30 pm and stay until 5:15 pm.

I understand that the ELO Program has an attendance requirement. All curriculum is taught progressively; therefore, your student(s) must be present to maintain their ability to participate. Once a student has three unexcused absences, they may be exited from the program.

Please consider other outside activities prior to submitting your interest. Attendance is essential.

I understand that the ELO Program will include academic enrichment, leadership training, collaborative activities, hands-on science, coding, STEM activities, drama, physical fitness, art, and more. Your student(s) will participate in ALL scheduled activities.

Our mission is to introduce your student to new activities, ideas, and curricula. We will ask you to partner with us and hold your student(s) accountable to engage in ALL activities. No exceptions.

I understand that the ELO Program MAY travel by school bus to a local gym and/or facility.  *

I understand that the ELO Program will receive training from i9 Sports one day a week. The ELO staff will support the expert i9 coaches in teaching sports skills, drills, teamwork, and sportsmanship. You will be responsible for completing a registration (it is free), and students are expected to participate in ALL activities.


A doctor's note will be required for non-participation. Often, the unknown or lack of practice has a student(s) wanting to refrain from participating. However, once they do, a new avenue of fun opens up. Let's support their journey to discovering new skills!
