SF Ballet & SFMOMA COLORFORMS Field Trip Ticket Order Form 
In partnership with San Francisco Museum of Modern Art (SFMOMA) Education, San Francisco Ballet is thrilled to announce an exciting free field trip opportunity for Bay Area schools. Join SF Ballet and SFMOMA for an educational screening of the dance film COLORFORMS, created by choreographer Myles Thatcher and filmed on site in the galleries of SFMOMA, Yerba Buena Gardens, Golden Gate Park, and the SF War Memorial Opera House. 

Each 60-minute screening includes an introduction from the choreographer, interactive standards-based dance education activities led by SF Ballet teaching artists, and a post-screening Q&A with artists, plus free access to the SFMOMA galleries where students can explore the works of art that inspired the choreography in the film. This field trip is an excellent opportunity to expose students to dance, music, media and visual art forms in a hands-on, interactive, and integrated setting. Recommended for grades 2-12. 

COLORFORMS experience screening dates and times:

**ALL COLOFORMS SCREENINGS ARE CURRENTLY FULL** - Schools may still complete the form but please note that you will be added to a WAITLIST and will only be assigned to a screening if space opens up. You will be contacted via email if we are able to move your group off of the waitlist and into a screening time.

  • Friday, May 17, 2024 @ 10:30-11:30 a.m.  (FULL) and 12:15-1:15 p.m. (FULL)
 *This screening is ideal for SDC classes and students with autism, sensory input disorders, sensory sensitivities, or other physical, cognitive, or developmental disabilities. Seating is limited. 

All the above screenings take place at SFMOMA's Phyllis Wattis Theater, located at 151 3rd St, San Francisco, CA 94103, and include self-guided SFMOMA gallery access for all classrooms attending. 

School groups attending COLORFORMS may also visit the museum's galleries at no cost to continue with their arts themed field trip. We recommend allowing an additional 45–60 minutes for your class to enjoy the galleries. Morning screenings include gallery access after the film; afternoon screenings have the choice between gallery access before or after the film, depending on school schedule. School groups who choose to visit the galleries before an afternoon screening should indicate so on the ticket order form and must plan to arrive at the Third Street entrance at 11 am on the day of their screening.

Seating requests will be filled in the order they are received; space is limited. We kindly ask that school groups indicate their 1st, 2nd, and 3rd choice date and time so that we can best accommodate everyone. Once your request has been processed, your ticket confirmation will be sent via email within 7-10 business days. 

School groups are responsible for arranging their own transportation to and from SFMOMA. SFMOMA offers a limited number of bus scholarships for SFUSD Title I schools. For more information please contact schoolvisits@sfmoma.org

Ticket orders will be fulfilled in the order they are received.
