Middle School Youth Risk Behavior Survey - Opt Out Form

In person grade 7 and 8 students will be taking the Middle School Youth Risk Behavior Survey during the week of April 19th. You can access the survey questions here: https://nccd.cdc.gov/youthonline/App/Default.aspx?SID=MS

This is an anonymous survey and survey results will be pooled with other Southern NH middle schools. The results from the survey will allow our school and community organizations to better support the development of our youth through education and programing.

If you'd prefer that your child does not participate in the survey, please complete the below form by Friday, April 16th.
