Parental Notification/Opt-out Form – School Climate Survey: High School
The School Climate Survey for high school is an anonymous survey used to identify school climate issues and prevention areas within our school. The survey for high school students includes 16 questions and should take no more than 15 - 20 minutes to complete in the first period class. The survey is anonymous, but parents and or guardians have the option to opt-out if desired. Fill out this form if you wish your child not to participate in this climate survey. Please hand this in before February 21 2022.
