Please provide the following information to receive bounty rewards from Treasury Guild. 

Please be aware that this links your wallet address with your Discord or Github name in a relatively public way. To protect your privacy, you might want to create a new wallet, which only has Ambassador Program transactions in it, and submit that.

This is a single submission form, if you send a second address it wont be updated in the system. The database only saves the first wallet you submit. This Google Form does not support overriding intial information automatically. If you ever want to change your wallet address please ask for assistance in this channel from treasury guild members. You can also attend a Treasury Operations meeting and we'll update your wallet in the system while building the transaction.
