It is very important that if your child has a medical condition the school is given as much information as possible.

Please go through and complete this form, there are the following sections:

EMERGENCY CONTACTS
PARENT/CARER AGREEMENT

Once this form is submitted it will be sent directly to our school office for processing.

Please ensure that if your child does require medication at the school, this is brought to the school office as soon as possible.  If your child requires an epipen we will require 2 of these to be kept on the school premises.
