Consent for Rapid Testing
RSD is seeking your consent to test your child for COVID-19, in the event that your child is potentially exposed to a positive or are exhibiting covid-like symptoms. If you consent, your child may receive a free diagnostic test for the COVID-19 virus that will be administered by one of trained nurses. Testing may be recurring and this consent will last for the 2022-23 school year.
A rapid COVID-19 test will be used, which will involve inserting a small swab, similar to a Q-Tip, into the front of the nose. (We are not using the long-swab test that goes to the back of the nose).
We will notify you immediately if your child tests positive for COVID-19. Any students who test positive will be sent home and must be kept at home for a minimum of five days and once they are fever free without the help of medicine.
Please be advised that the law requires and/or allows some information about your child to be shared with Monroe County and New York State Public Health Agencies. This includes notifying the State Health Department about positive COVID-19 results, including the student’s name, date of birth, race, ethnicity, gender, address, phone number, and result of the COVID-19 test.
PARENTS AND GUARDIANS: PLEASE COMPLETE A SEPARATE FORM FOR EACH OF YOUR CHILDREN.
