If you chose graphic color placement option 1, please write in 1 color number in the space provided below. If you chose option 2 please write in 2 color numbers (please specify which color is for your desired word and which is for the background). If you chose option 3, please write in 3 color numbers (please specify which color is for your desired word, which is for the flower and which is for the background). If you chose option 4 (please specify which color is for the stem, which is for the flower and which is for the background). Lastly, if you chose option 5 please write in 4 color numbers (please specify which color is for your desired word, which is for the color of the stem, which is for the flower and which is for the background).

Note that the fewer letters/characters your word/phrase contains, the larger the type size is able to be.

Question 5: Your piece comes ready to hang with 2 silver grommets that I install in the top two corners of your piece. If you would like two additional grommets installed on the bottom 2 corners please check below. Thanks

Select which size artwork are we creating for you from the options below. Please note this is your final cost which includes all design fees, materials, shipping as well as 25% going to the charity of your choice

Option 1: 18" x 36". $79.00

Option 2: 24" x 48" (standard size). $99.00

Option 3: 36" x 72" $149.00

Option 4`: 36" x 96" $189.00

Question 6: To whom and at what address should the art be shipped to, or delivered via another arrangement? *

If you wish to have it shipped to another person as a Forever Flower Gift please give me their name, mailing address and telephone number.

Question 7: 25% of the purchase price goes to the charity of your choice, please write in the name of the organization you wish to support in the space below and I will send you a confirmation. *

I will send you a "proof" of what you have created before your artwork goes into the printing phase! You will get to see it before I make it. Any other questions or thoughts please share below.
