The first day of Quest 2023/24 Season is 8/14/23. If your child has not yet started school at that time, we encourage you to attend this first week of tutoring anyway. It is an excellent opportunity for your child to meet his/her tutor and work to establish a study routine and organization systems for the new year.

I hereby release, acquit and agree to hold harmless, Quest Academy, it’s agents, instructors, successors, and assigns, from any liability or claims of any kinds whatsoever, resulting or arising from my participation in any programs, classes, camps or any other function otherwise offered or sponsored or participated in by the Academy, including, but not limited to, the participation therein by my child(ren). *
Because our schedule is extremely tight, please arrive on time to pick up your child. I understand that emergencies arise at times, but there will be a $10 late charge for students left in excess of 10 minutes unless previous arrangements have been made. *
I have read and understand the pricing policies explained in the Welcome Back letter. I understand:
Payments are due prior to the first session of the month.
Credits will not be given for missed sessions and while we will try to schedule make-ups, they are not guaranteed.
The monthly fee is the same each month, regardless of whether there are 3, 4, or 5 weeks in the month.

I understand that my child will remain on the schedule and I will be invoiced as such, until Quest receives in writing my desire to cancel further sessions. Cancellation requests require 30-day notice. I also understand that as sports schedules become available, Quest will work with me to adjust our schedule. *
I agree to switch my child's session to Zoom and keep my child home if he/she is ill.

We will make our best effort to keep your child on a set schedule. However, on occasion, your student may work with a different tutor. If this happens, there will be direct communication with your child’s substitute, always a Quest employee, as to your student’s needs and lesson plans. *
