Liability and Media Waiver

I give permission for above named minor child to participate in RVCH Training Sessions. I realize that said activity involves the potential for injury which is inherent in all sports. I knowingly release and hold harmless Rainbows Volleyball Club Hawaii – RVCH and all of its Representatives, Coaches, Volunteers, and Assistants, from any and all liability. I accept all responsibility for all financial obligations which may result from injuries sustained while participating in the Clinic. I authorize RVCH representatives to take any necessary steps for required medical treatment for the above named participant and release RVCH from any and all liability for any medical treatment decisions for said participant. I give consent for RVCH to use my child’s name, video, photograph, for all RVCH media purposes.
