Please complete the form to request an ACT or SAT Waiver. Please note: Only Juniors and Seniors who qualify for free or reduced lunch are eligible for fee waivers. You can receive a total of two ACT waivers and two SAT waivers. If you lose your fee waiver it cannot be replaced.
